Terms of Service
The agreement between you and Vellform. Plain language, and specific to what this product actually does.
Last updated: [[ OWNER TO SUPPLY — termsLastUpdated ]]
1. This agreement
These terms are between you and [[ OWNER TO SUPPLY — legalEntityName ]] (“Vellform”, “we”). By creating an account or using the service you accept them. If you are accepting on behalf of an organisation, you confirm you are able to bind it.
2. Your account
- You must be at least [[ OWNER TO SUPPLY — minimumAge ]] years old.
- Give accurate details, and keep your password to yourself.
- You are responsible for everything done under your account, including by anyone you give access to.
- Disposable email addresses are rejected at sign-up.
- One person, one account. Do not create accounts to obtain extra free credits.
3. What you may not use Vellform for
Vellform builds forms, and a form is a good tool for asking people for things they should not give you. So this list is specific rather than general. You may not use Vellform to:
- Harvest credentials. No forms that ask for passwords, one-time codes, banking credentials or full card numbers.
- Impersonate. No forms that pretend to be a bank, a government body, an employer or any organisation you do not represent.
- Deceive. No fake prize draws, fake surveys or misrepresented purposes.
- Send unsolicited bulk messages linking to your forms.
- Collect data about people who have not been told why you are collecting it.
- Upload or solicit unlawful material, or anything that infringes somebody else's rights.
- Probe, scrape or overload the service, or work around rate limits and plan allowances.
- Resell Vellform as your own product without a written agreement with us.
4. Your forms and your responses
You own them. Your forms, your branding and every response you collect remain yours. We claim no ownership and we do not use your content to train models.
You give us the permission we need to run the service — to store your content, to display it to the people you share it with, and to send it to the providers listed in the Privacy Policy. That permission exists only to operate Vellform and ends when you delete the content.
5. Your duties to the people who answer
For the responses your forms collect, you are the data controller and we process them on your instruction. That makes some things your responsibility, not ours:
- Having a lawful basis for asking what you ask.
- Telling respondents who you are and what you will do with their answers.
- Answering their access, correction and deletion requests.
- Knowing that multi-page forms save progress before submit, so an abandoned form still leaves a partial response you can read.
6. Credits
AI work is metered in credits. Reading your form, opening it, and every non-AI feature cost nothing; a credit is charged when the assistant does work.
- Each plan includes a monthly allowance — 25 on Free, up to 1100 on Premium.
- Unused credits do not carry over. The allowance resets each billing period.
- You can buy more at any time: a top-up pack of 250 credits for $18. Purchased credits sit on top of your allowance and are not cleared by the monthly reset.
- When you run out, AI requests stop until the allowance resets or you buy more. Your forms keep working and keep collecting responses.
- If overage billing is enabled on your account, work beyond the allowance is charged at $0.05 per credit. It is off by default and is never switched on without your action.
- Credits have no cash value and cannot be transferred or refunded as money.
7. Plans, billing and cancellation
- Paid plans are billed in advance, monthly or annually, by Dodo Payments as merchant of record. Your card details never reach us.
- Prices are in US dollars and exclude any tax that applies where you are.
- A subscription renews automatically until you cancel. Cancelling stops the next renewal; it does not refund the period you are in.
- On cancellation you keep the plan until the period ends, then return to Free.
- We may change prices. Existing subscribers get notice before a change affects them.
8. Refunds

9. AI output
The assistant is a tool, not an author with judgement. Check what it produces before you publish it. It can misread a brief, ask a question you did not intend, or write a label that is wrong for your audience — and a published form is your responsibility, not ours.
We make no promise that its output is accurate, complete, lawful in your jurisdiction, or fit for a particular purpose. Two identical prompts may produce different forms.
10. Availability
We work to keep Vellform running, but we do not offer a service level agreement or an uptime guarantee, and the service may be unavailable during maintenance or because a provider we depend on is down.
11. Suspension and termination
We may suspend or close an account that breaks section 3, that puts the service or its users at risk, or whose payment fails and is not resolved. Where it is reasonable to do so we will tell you first and give you a chance to put it right.
You can close your account at any time from Settings. Your subscription is cancelled straight away, and your workspaces are scheduled for deletion. For 30 days the deletion can still be undone: sign in again and it is cancelled automatically. After that window everything is erased permanently — your forms, responses, uploaded files and exports — and cannot be recovered, because we keep no backups to recover from.
12. No warranty
Vellform is provided “as is”. To the extent the law allows, we exclude all implied warranties, including merchantability, fitness for a particular purpose and non-infringement.
13. Limitation of liability
To the extent the law allows, we are not liable for indirect, incidental or consequential loss, for lost profits, or for lost data.
[[ OWNER TO SUPPLY — liabilityCap ]]
Nothing here excludes liability that cannot lawfully be excluded.
14. Changes to these terms
We may update these terms. Material changes will be reflected in the date at the top, and continuing to use Vellform after that means you accept them. If you do not, close your account.
15. Governing law
These terms are governed by [[ OWNER TO SUPPLY — governingLaw ]], and disputes will be heard in the courts of that jurisdiction.
16. Contact
[[ OWNER TO SUPPLY — legalEntityName ]], [[ OWNER TO SUPPLY — registeredAddress ]]. Write to [[ OWNER TO SUPPLY — contactEmail ]] or use the contact page.
These terms are not ready to publish. The operator details above are unset — fill them in at src/lib/site/site-identity.ts. Have a qualified adviser review this document before you rely on it.
